permanent full-time and part-time Nurse Practitioners (NPs)
to support daily patient care, visits, and the implementation of clinical programs that promote optimal patient health and outcomes.
The Nurse Practitioner will provide care to patients rostered to
Queen Square physicians
as well as to patients of
partnered community physicians
within our collaborative network.
Reporting to the
Manager of Nursing Services
, the Nurse Practitioner will work within an
interdisciplinary team
to support partnering physicians and their patients, develop and deliver clinical programs, and provide direct care in key areas such as
seniors' health, gender health, palliative care, and chronic disease management
.
Working to the
full scope of practice
, the NP will independently assess, diagnose, and manage patient care within the practice.

some evening or weekend coverage may be required

Responsibilities
Provide acute and episodic care in the clinic and offer home or virtual visits as appropriate
Complete health assessments, including a health history and physical examination, formulate and communicate medical diagnoses and develop care plans and case conferences
Monitor the ongoing therapy of patients with chronic stable illness by providing effective pharmacological, complementary or counseling interventions
Work collaboratively with other team members to manage the needs of patients in Senior's Wellness, Palliative Care, and Gender Health and various other programs.
As appropriate, share results of assessment and intervention with physicians, team members, hospital, home care, and others in the circle of care
Liaise on and collaborate with families and community stakeholders
Develop and evaluate educational resources, presentations, workshops, for the internal team and for patients and their families
Recommend and arrange for specialist referrals and appointments in consultation with physicians and team members
Exercise reasonable care and caution in protecting confidential and sensitive information related to clients and personnel
Maintain relationships with networks, related professional organizations and other related service providers
Perform other related duties as assigned by the Manager of Nursing Services

Qualifications
RN(EC) licence from College of Nurses of Ontario in good standing, designation required
Completion of Nurse Practitioner (NP) Primary Care certificate is required
Master's degree in Nursing is required
Recent related nursing experience; Prefer three years working experience in a community-based or hospital-based clinical team delivering related programs
BLS/CPR
Proof of professional liability insurance
Experience working as part of an interdisciplinary team
Comfortable with electronic documentation and use of technology in the workplace
