Position Overview:

We are seeking a dedicated and experienced

Nurse Practitioner (NP)

to join our healthcare team. The NP will provide primary care services, conduct assessments, diagnose conditions, develop treatment plans, and collaborate with other health professionals to improve patient outcomes.

Key Responsibilities:

Conduct comprehensive patient assessments and histories. Diagnose, treat, and manage acute and chronic illnesses. Order and interpret diagnostic tests. Prescribe medications and therapies in accordance with Ontario regulations. Provide health promotion, education, and disease prevention strategies. Collaborate with physicians, pharmacists, and other health team members. Maintain accurate and timely electronic medical records.

Qualifications:

Current registration in good standing with the

College of Nurses of Ontario (CNO)

as a Nurse Practitioner. Primary Health Care NP (PHC-NP) designation preferred. Strong clinical assessment and diagnostic skills. Excellent communication and interpersonal skills. Commitment to patient-centered care. Experience in a family practice or community health setting is an asset.
