:

The Unattached Patient Clinic at Huronia Urgent Care Clinic (HUCC) is a pilot initiative led by the HUCC< RVH, and the Barrie and Area Ontario Health Team (BAOHT) to improve access to care for unattached patients in the region. The Nurse Practitioner (NP) will play a key role in providing comprehensive, patient-centered primary care follow-up to individuals without a family physician or nurse practitioner that have been referred to the Clinic.


Working with an interprofessional team, the NP will focus on:


Managing chronic conditions Providing preventative and routine screening Coordinating referrals and follow-up care Reducing unnecessary emergency department (ED) visits

This position offers a unique opportunity to shape an innovative care model that addresses system gaps, enhances equity of access, and improves outcomes for vulnerable patients


Responsibilities Include:




Conduct in-depth assessments of unattached patients, integrating patient history, diagnostic reasoning, and interprofessional input Initiate and evaluate medical and nursing therapies within the NP scope of practice, applicable directives, and clinical guidelines Provide ongoing management of chronic illnesses, preventive care, and routine health screening Collaborate with physicians, community providers, and Ontario Health partners to ensure coordinated care and smooth navigation of the health system Provide health education and coaching to patients to improve self-management and health outcomes Participate in quality improvement, evaluation, and reporting activities related to this pilot clinic Support the development of best practices for unattached patient care models Contribute to an environment that values equity, inclusion, cultural safety, and patient-centered care

Qualifications:




Education/Certification: Master in Nursing/Nurse Practitioner (MN-NP) or Master of Science in Nursing with successful completion of a post Master's Primary Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required Current certification in Basic Cardiac Life Support (BCLS) required

Experience




Minimum three (3) years of clinical experience in primary care, urgent care, community health, or chronic disease management Demonstrated knowledge of health promotion, disease prevention, and management of acute and chronic conditions Methodology, discharge planning and community resources required Previous experience working with vulnerable or underserved populations preferred Experience in patient education, program development, and evaluation considered an asset

Proficiency in electronic medical records (EMR) and standard office applications Satisfactory criminal reference and vulnerable sector screening prior to hiring * Flexibility to adapt within a pilot program environment
