As part of the Cardio-Respiratory Department, the Heart Function Clinic (HFC) is a Nurse Practitioner/Cardiologist-led clinic providing guideline-directed, technology-enabled, and coordinated care for patients with heart failure.
As the Nurse Practitioner (NP) in the HFC, you are a skilled heart failure clinician responsible for delivering care in alignment with the Vitally You Strategy and RVH's Strategic Goals. The HFC NP models clinical excellence, adheres to the College of Nurses of Ontario (CNO) practice standards, and follows RVH policies and procedures.
In this advanced practice role, you will demonstrate autonomy and expertise in managing acute and chronic illness, promoting wellness, and making complex clinical decisions. The clinic provides specialized management of patients requiring outpatient intravenous diuretics, advanced heart failure therapies, amyloidosis care, and palliative care support. The principal responsibilities include utilizing and demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for patients involving collecting and interpreting data about the health of the patient; determining a medical diagnosis and treating complex responses of individuals and families to actual or potential problems; performing quality improvement and administrative responsibilities; participating in policy and procedure development; performing research activities; delivering, coordinating and participating in education and professional development activities/programs; performing activities to further develop and strength the profession; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies; performing cross-functional and/or other duties consistent with job classification, as assigned or requested.
