Why is this position important?




The Nurse Field Case Manager will respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, and effective problem-solving abilities, both in a team environment as well as independently. Building strong working relationships both internally and externally are key responsibilities to meet specific team and business objectives. In addition, you will attend pre-booked training sessions, complete self- learning assignments and independent studies while adhering to SDM Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice.

What you will do:



Responsible for assigned on caseload Act as the primary resource & contact for patients, physicians, Health Care Professionals and vendors within the designated area. Coordinates & facilitates the client progress from enrollment to drug administration and throughout therapy. Facilitate self care through the tenets of advocacy, shared decision-making and education. Practice cultural competence, with awareness and respect for diversity. Ensure thorough documentation in patient file to ensure continuity of care is maintained and that any changes in patient care are clearly identified and transitioned accordingly. Provide status updates related to reimbursement; coordinate nursing requirements; blood work; pharmacy dispensing / delivery / refill requests. Report all Adverse Events (AE) and Product Technical Complaints (PTC) immediately or as soon as possible upon becoming aware of the event as per current policies and procedures Promotes communication and coordination between the MDs/pharma/patients/clinic staff and other departments within SDM Specialty Health Network Respect confidentiality, advocate for patients/families to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian Nurses Association Code of Ethics Participate in internal and external committees. Plan and implement in-services and continuing education programs for both internal and external stakeholders as requested by management. Demonstrate commitment to Continuous Quality Improvement. Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances. Participate in ongoing reflective practice activities. Assume responsibility for personal and professional development. Other responsibilities as required by SDM Specialty Health Network This role will provide in-person service to patients and/or prescribers, or remotely as needed according to business requirements.

What you will need

Five or more years direct clinical experience Current professional nursing license in the province of work Three to five years of experience in oncology or hematology care Fluently bilingual in both written and spoken languages (French & English) BScN preferred, however, a combination of experience and education will be considered Ability to provide an organized, efficient approach to case management Possession of excellent skills in assessment, communication, critical thinking and decision making Ability to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethics Previous experience with Health Canada Guidelines for reporting adverse events is an asset Proficiency of computer skills including: Microsoft applications (Outlook, Word, Excel and Internet Explorer); experience with web-based applications (Salesforce) Ability to present to fellow colleagues Ability to remain objective and be a team player Ability to adapt to a changing environment Commitment to the principle of quality client-centered service delivery Knowledge of theory and implementation of Continuous Quality Improvement processes Knowledge of case management processes Valid driver's license

Responsable de la charge de travail des patients Agir en tant que ressource principale et contact pour les patients, les medecins, les professionnels de la sante et les fournisseurs dans le domaine designe. Coordonner et faciliter la progression du patient, de l'inscription a l'administration de la medication tout au long du traitement. Faciliter le developpement d'une routine de soins personnels grace aux principes de la defense des interets, le partage de la prise de decisions et l'education. Exercer vos competences culturelles en faisant preuve de sensibilite et de respect envers la diversite. Veiller a ce que la documentation fournie dans le dossier du patient soit complete afin de garantir la continuite des soins et que tout changement dans les soins du patient soit clairement identifie et que la transition soit organisee en consequence. Fournir des mises a jour des statuts ayant trait au remboursement; coordonner les exigences en matiere de services infirmiers; gerer les analyses sanguines et la preparation, la livraison et les demandes de renouvellement des ordonnances avec la pharmacie. Signaler immediatement ou des que possible tous les evenements indesirables (EI) et plaintes techniques sur le produit (PTC), conformement aux politiques et procedures en vigueur. Faciliter la communication et la coordination entre les medecins/pharmaciens/patients/le personnel de la clinique et les autres services au sein du Reseau de sante specialise SDM. Respecter la confidentialite, agir comme defenseur des interets des patients et de leurs familles, pour faciliter des resultats positifs, et veiller a reconnaitre et resoudre les problemes de nature ethique conformement au Code de deontologie de l'Association des infirmieres et infirmiers du Canada. Participer a des comites internes et externes. Planifier et mettre en application des programmes de formation sur place et en continu, a l'intention de parties interessees tant internes qu'externes, selon les demandes de la direction. Demontrer un engagement envers l'amelioration continue de la qualite. Maintenir des normes elevees en matiere de pratique professionnelle en adherant a l'ethique de la profession et en se tenant au courant des dernieres percees sur le plan professionnel. Participer a des activites de pratique de reflexion en continu. Assumer la responsabilite du developpement personnel et professionnel. Prendre en charge d'autres responsabilites selon les besoins du Reseau de sante specialise SDM.

Qualifications

requises



Une experience clinique directe de cinq annees ou plus Parfaitement bilingue a l'ecrit et a l'oral (francais et anglais). Trois a cinq ans d'experience en soins oncologiques ou hematologiques Etre titulaire d'un droit d'exercer la pratique infirmiere dans la province de travail Baccalaureat en sciences infirmieres prefere; cependant, une combinaison d'experience et de formation sera consideree Faire preuve d'un leadership reposant sur l'experience Capacite a proposer une approche organisee et efficace au gestionnaire de cas Excellentes aptitudes en evaluation, communication, pensee critique et prise de decision Aptitude a comprendre les politiques, les aspects juridiques et l'ethique professionnelle du Reseau de sante specialise SDM Une experience anterieure avec les directives de Sante Canada pour le signalement des effets indesirables Excellentes aptitudes en communication en anglais (bilinguisme francais/anglais si necessaire) Des competences en informatique, y compris avec : Applications Microsoft (Outlook, Word, Excel et Internet Explorer); experience avec les applications Web (Salesforce) Capacite a faire des presentations a ses collegues de travail Capacite de demeurer objectif et d'avoir un bon esprit d'equipe Capacite d'adaptation dans un environnement en mutation Faire preuve d'engagement quant au principe de prestation de services de qualite axes sur les clients Connaissance de la theorie et la mise en application de processus d'amelioration de la qualite en continu Connaissance des processus de gestion de cas Permis de conduire valide Ce role fournira un service en personne aux patients et/ou aux prescripteurs, ou a distance selon les besoins de l'entreprise.
