Nursing Coordinator



Reporting to the Executive Director, the Nursing Coordinator is an integral member of the CNRO team Utilizing nursing assessment and clinical decision making along with your background of nursing knowledge in acute or community settings, you promote an excellent patient experience by acting as a clinical resource to the care team, being a liaison between the care team, the patient and their family.

The ideal candidate is

? Passionate about nursing and patient care.

? A Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario

? A current member of the RNAO or RPNAO

Roles and Responsibilities:

? Provide potential new clients with information about CNRO practices, fees and following up on calls for information.

? Obtain a history of the health care needs and guide the client to determine the level of care needed and the frequency of visits.

? Initiate care plan development based on the intake history, communicating the needs and priorities to the members accepting the assignment.

? Source members and set up initial schedules.

? Assist in clinical decision making and care planning as needed.

? Ensure monthly reporting from members is current, follow up on client issues, suggest nursing interventions as needed.

? Maintain ongoing contact with clients to maintain a current record of health status and care needs; ensure changes are recorded and communicated to all members of the care team.

? Coach members as needed on interventions, therapeutic communication and clinical skills.

? Work collaboratively with the Nursing Coordinator team to maintain seamless access to information and support for clients and members.

? Have a knowledge of and adhere to the College of Nurses of Ontario Standards of Practice and act as a resource for members.

Hours

? Part-time 1-2 days per week, 8 hours per day.

? After hours on call 1-2 days per week associated with the scheduled days.

? 1-2 weekends per month

? The Coordinator team self-schedules ensuring full coverage of all scheduled days and on-call shifts.

Pay: $35.23-$43.98 per hour

Expected hours: 8 - 16 per week
