Our Clinical Care Coordinator (CCC) is the face of our innovative house call clinic and navigates our patients and families through the complexities of the healthcare system.

We are searching for a dynamic and empathetic Clinical Care Coordinator to act as the guide for our patients' care. You will be the primary contact for patients and families, liaise with our MDs, PT, frontline HCAs, and outside allied health members to create tailored care plans. What You'll Do: Your Impact



As the heart of our care delivery, you will:

Serve as the Lead

: Act as the trusted, primary point of contact for patients and their families, listening to their concerns and advocating for their goals.

Architect the Care Plan:

Collaborate with physicians and allied health members to integrate the medical plan into the patient's personal goals.

Lead the Team:

provide guidance, feedback and education to the care aides on our team to implement care plans with professionalism and compassion.

Solve Problems Creatively:

Proactively identify potential barriers to care--be they logistical, emotional, or financial--and use your critical thinking skills to find innovative solutions.

Manage with Empathy and Skill:

Build strong, empathetic relationships while expertly setting realistic expectations and maintaining professional boundaries to ensure the best outcomes.

Spread the Word:

Support the team in our outreach efforts to educate other community groups about our innovative clinic and nurture existing relationships with regular contact.

Adapt and Evolve:

Recognize that a patient's journey changes over time and skillfully adapt the care plan to meet their evolving health status and goals.

A successful candidate will require:

A valid Alberta nursing license. Experience managing HCAs or other healthcare professionals Prior experience with health care case management Nursing experience in the community setting A valid driver's license and vehicle Comfort with using computers and online platforms

Pay: $70,000.00-$90,000.00 per year
