ABOUT THIS ROLE:

The Nurse Case Manager (NCM) will be the primary point of contact offering clinical support to patients and HCPs. The NCM will provide holistic support to patients to help them manage their medication and their disease, considering their individual needs and motivations. The NCM is skilled at uncovering when to provide the right information, in the right amount, at the right time. The NCM embraces the concept of shared decision-making with the patient, and acknowledges and leverages patient\xe2\x80\x99s past experiences and strengths. The NCM will provide support that is complementary to the support provided by the patient\xe2\x80\x99s healthcare team. The NCM will also act as a resource and liaison to the patient\xe2\x80\x99s healthcare team.

A DAY IN THE LIFE:

  • Develop strong relationships with patients, caregivers, and their healthcare team by infusing confidence, trust, security, and relief
  • Complete outbound and inbound calls to patients, HCPs, and other key players to support patients through their treatment journey.
  • Assist patients in identifying their needs and establishing motivating goals on their way to finding a healthful new normal as they integrate their new treatment into their day-to-day life.
  • Assumes a strengths-based approach in coaching the patient to a higher level of self-efficacy, including greater adherence and persistence on treatment.
  • Assist patients in managing treatment side effects
  • Develop a strong knowledge of the care provided at the patient\xe2\x80\x99s clinic/cancer centre to complement the support provided to the patient
  • Relay information back to the patient\xe2\x80\x99s healthcare team as agreed upon with the healthcare team and respecting patient confidentiality
  • Refer patients to community resources, including patient organizations, as well as identifying additional potential resources to program management
  • Uncover and proactively share gaps and opportunities for improvements in patient care
  • To be a proactive catalyst for the continuous improvement of the experience offered to patients and their caregivers
  • Diligent record-keeping to ensure continuous care and experience.
  • Act as a key resource liaison for program stakeholders, which include physicians, nurses, patients, the client, drug access navigators and clinics.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Report and document Adverse Events as per Pharmacovigilance requirements.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company\'s Quality Management System as well as specific performance indicators for the project.
  • Participate in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Ability to travel as required.
  • Complete other tasks as requested.

Qualifications

WHAT YOU BRING TO THE TEAM:
  • Registered and in good standing with the College of Nurses
  • 2+ years of experience working with oncology patients with considered a strong asset
  • Ability to assess patient needs utilizing evidence-based assessment tools
  • Demonstrate sensitivity and commitment to utilizing a patient centric approach; is adept at responding to the patient\xe2\x80\x99s emotional, educational, and health literacy needs,
  • Demonstrates cultural competency.
  • Is comfortable with a more collaborative partnership with a patient, rather than a directive approach
  • Must have a proven track record with excellent customer services skills internally as well as externally
  • Experience working with Patient Support Programs
  • Excellent verbal and written communication skills and interpersonal skills (French is an asset)
  • Reimbursement experience (asset)
  • Understanding of provincial and private coverage criteria and how to navigate the Canadian drug coverage landscape
  • Familiarity with the Special Authorization process for both public and private insurance in assigned territory.
  • Strong Computer and Technical skills: Excel, Word, Outlook, tablets, web-portals/internet use
  • Making decisions using sound judgment, and meeting deadlines.
  • Ease with working in a paperless environment.
  • Proven ability to work in a team environment
  • Strong sense of organization and attention to detail
  • Self-regulation of time management and the ability to multi-task and adhere to deadlines
  • Familiarity or practice with Motivational Interviewing techniques, or any other kin
