Your Role as a Nurse Case Manager:

You'll be the key point of contact in coordinating holistic care plans tailored to each client's unique needs, ensuring care is not just effective--but empowering.

Responsibilities:

Collaborate with clients, families, and healthcare professionals to develop personalized care plans. Manage client cases from intake to discharge, including assessments and follow-ups. Ensure safe, high-quality, and client-centered care delivery. Mentor and support staff, conduct regular performance reviews, and hold team meetings. Provide ongoing communication and training to care staff about client needs and changes. Support supply management, PPE distribution, and care coordination logistics. Maintain accurate and timely documentation for all client-related activities.

What You Bring:

Experience in community health is an asset An active LPN license with BCCNM, with no restrictions 1-2 years supervisory skills. This includes having experience with decision making, coaching, and negotiation Familiarity with the quality management approach and commitment to customer service. Valid driver's license Current criminal background check will be requested
