Responsibilities
Deliver advanced clinical expertise in the assessment, planning, implementation, and evaluation of care for clients with wound, ostomy, and continence needs.
Collaborate with families and the interdisciplinary care team to develop and maintain individualized care plans.
Educate and support clients and caregivers on wound, ostomy, and continence management.
Serve as a clinical resource and mentor for nursing colleagues within The Care Company.
Maintain accurate documentation in compliance with professional and organizational standards.
Contribute to quality improvement initiatives and best practices in client care.

Requirements
Registered and in good standing with the College of Nurses of Ontario (CNO).
Wound, Ostomy and Continence Education Program (WOC-EP) from NSWOCC* Preference will be given to candidates who have completed any of the following: (1) a master's program in Advanced Wound Care (MScCH WPC) from UofT, or (2) a Master of Clinical Science (MCISc) in Wound Healing Program from Western University or (3) an education program with the International Interdisciplinary Wound Care Course (IIWCC) from UofT.
RN diploma/degree from a recognized university or college.
A moderate level of wound care or venipuncture knowledge and experience is an asset.
Knowledge of community nursing is an asset.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
A valid Ontario driver's license (G License) and access to a reliable vehicle is an asset.
Current CPR and First Aid certification
, completed through a recognized training provider within the past 12 months.
Valid WHMIS 2015 certification
, issued or renewed within the past 12 months.
Negative TB test
(2-step or 1-step with prior documentation), completed within the past 12 months.
Proof of full COVID-19 vaccination
, or exemption in accordance with the Ontario Human Rights Code.
Clear Criminal Record Check with Vulnerable Sector Screening
, issued within the past 12 months.
Must be able to
communicate effectively in English
, both orally and in writing, for documentation and communication with clients and care teams.
Must be comfortable and capable of providing care to both male and female clients
