8 hour shifts, Saturday, Sunday Days @Midtown
Additional 16hrs (4 and/or 8 hour shift) Monday-Friday Days @Chicopee
Position Summary:
A unique opportunity to support both campuses in inpatient and outpatient service areas. As a member of a multi-disciplinary Collaborative Practice Team, the Weekend +1 Weekday Coverage Social Worker (0.6 FTE) provides assessment, treatment, support, and service coordination to patients and their families throughout their hospital stay and assists with transition back to the community. As a member of a multi-disciplinary team, the outpatient specialized Rehabilitation Social Worker (0.4 FTE) provides assessment, treatment, support, and service coordination to patients and their families. They also provide or facilitate care for patients with mental health concerns who often have unique challenges in accessing and receiving treatments. The successful candidate will participate in program planning/development; professional and hospital initiatives supporting quality management and research; mentoring of colleagues and students; and assist with coverage across other clinical areas as required. This position currently supports social work practice and discharge planning on weekends @Midtown and outpatient neurorehabilitation and pulmonary rehabilitation clinics on weekdays @Chicopee, but can be re-assigned based on organizational needs. Additional responsibilities: contributes to ensuring there is a safe environment for all patients, staff, and visitors.
