JOB PURPOSE
Working in collaboration with hospital care teams and community partners, the Palliative Clinical Nurse Specialist provides leadership and expertise that supports optimal palliative care in the hospital and community settings. This role includes the five components of a CNS: practitioner, consultant, educator, researcher, and leader. The balance of these components will vary depending upon the needs of the client and service requirement. Work is performed in accordance with Horizon Health Network administrative policies and procedures.
QUALIFICATIONS

• Master's degree in nursing
• Experience working in palliative clinical practice within the past 5 years in hospital and/or community-based settings.
• Excellent knowledge base of all aspects of palliative care
• Active registration or eligible for registration with NANB
• Canadian Nurses Certification in Hospice Palliative Care - CHPCN-c or willingness to obtain.
• Experience and training in LEAP facilitation and coaching
• Exceptional communication and interpersonal skills including experience in public speaking and media relations.
• Experience with Word, Excel, Power Point with an ability to manage and understand databases, statistical analysis, and graphic presentations.
• Ability to function independently and in a team environment.
• Demonstrated clinical practice that applies evidence-based knowledge to clinical situations with clients across the lifespan.
• Generalized knowledge of the palliative community population and available community resources.
• Knowledge of provincial palliative care strategy
• Demonstrated strong leadership skills.
• High regard for confidentiality and advocacy for patients
• Current certification in BCLS
• Excellent driving record, and ability to transport self to all work areas locally and provincially.
• Ability to attend work on a regular basis.
• Ability to perform the duties of the position.
• Good work record.

KEY RESPONSIBILITIES

• Practitioner: Demonstrates advanced practice through the reflective application of theoretical knowledge and clinical expertise. In collaboration with the interdisciplinary team, this expertise is focused on complex palliative client situations. Through expert clinical practice the specialist models and contributes to the evolution of standards of practice for all disciplines in a community and hospital setting and develops competency tools which address professional, ethical and legal palliative issues.
• Consultant: Collaborates with a variety of people including clients, families, healthcare professionals and community groups in the identification of needs and/or concerns. Acts as a consultant on issues related to palliative care practice. Actively assists in the development of policies and programs. Builds and maintains links with local community agencies and other centers of excellence to create possibilities for mutual advancement.
• Educator: Collaborates with the interdisciplinary team in the development of educational opportunities for clients and health professionals to promote independence, health, wellness, and prevention. Initiates and participates in wellness initiatives for employees. Networks with providers of health education and related programs at the regional, provincial, and national level. Acts as a role model, mentor, resource, and change agent in Horizon Health Network.
• Researcher: Models critical thinking and promotes systematic inquiry related to clinical practice. Promotes dissemination of evidence informed practice that supports optimal palliative clinical practice. Participates in research initiatives, projects, or reviews that impact client care and clinical practice. Participates in development of annual palliative care council quality reports, identifies relevant measurables and follows up with related initiatives.
