Who you are
You have a worked in a primary healthcare or health related setting for a minimum of 5 years. You have experience in conflict resolution, change management, leading and facilitating meetings, committees and workgroups. You have a high proficiency level in Microsoft Office applications. Written and spoken competence in English is required at an advanced level. Billingual would be an asset
What you will do
The PHCAC will function as a liaison between community-based PHC providers and the RHA. The PHCAC will develop business case and negotiate with the relevant departments within the RHA to enable particular services to be delivered in a community PHC setting and/or to assign from the team of providers who already work in an outreach capacity (i.e. outreach diabetes outreach case managers). The goal is to promote the sharing of processes and resources between clinical settings in an effort to optimize efficiencies in collaboration with other teams (i.e. move away from silos across settings).
The PHCAC will be responsible for supporting primary care practices (the latter as requested) to determine the "needs" of their practice population (i.e. meeting with teams to learn about their experiences of providing care to their practice population and leveraging relevant information/datasets from the RHA and DOH); this information will help to determine the potential staffing complement (allied healthcare providers) that may be required to create strategic community centered inter-disciplinary primary health care teams and/or primary health care \xe2\x80\x98hubs\xe2\x80\x99.
Facilitating implementation of outreach allied healthcare provider team(s) to partner with and provide services in private and community based primary care practices: plan, implement and evaluate specific priority quality initiatives to provide client access to education, self-management support, quality care and develop dissemination plan to support spread of leading practices/services across communities served within Horizon Health Network.
Facilitating and supporting practice changes: The PHCAC will be responsible for coordinating the logistical and change management supports required to assist PHC providers as they begin to shift their practice styles (i.e. from working as solo practitioners to working within collaborative, inter-disciplinary teams, providing extended hours of care and facilitating shared patient coverage, etc\xe2\x80\xa6). The PHCAC will coordinate the training and support required by these new teams including how to use advanced access scheduling, how to leverage EMRs, foster team building skills, etc\xe2\x80\xa6). Establishing mechanisms for inter-disciplinary communication, collaboration, negotiation, and conflict resolution will be critical.
Connecting orphaned patients to PHC providers: Within their respective health area, the PHCAC will be responsible for managing and monitoring the Patient Connect NB (PCNB) registry. This will include working with local medical staff offices and multiple PHC providers in an effort to link patients listed on the PCNB to family practice providers as quickly as possible.
Building relationships with stakeholders and community partners: The PHCAC will be responsible for developing and maintaining positive strategic relationships with stakeholders within the public, private and non-profit realms. The goal is to share information about community assets/resources, foster effective teamwork across communities/departments/services/programs with a view to improving clients access to community- and team-based services. The PHCAC will also establish mechanisms to involve relevant care providers, clients/families, and other stakeholders in the planning for development and direction of future PHC programming. This will require effective communication with key stakeholders to identify changing needs, resources and conditions in the community. The PHCAC will participate/lead relevant community meetings and relevant councils, roundtables, and committees.
Monitoring communication processes, workload, and reporting outcomes: The PHCAC will be responsible for developing appropriate practice processes and agreements required to capture the clinical workload and documentation of providers. The PHCAC will also monitor outcomes of service delivery (such as numbers of visits, costs/visit, use of acute care services, etc\xe2\x80\xa6) to ensure that the outreach clinicians are working within a model of care that is efficient and effective. The PHCAC will coordinate meetings with the outreach team and stakeholders/partners as required. The PHCAC will also be responsible for providing relevant data to their partners within the RHA and DOH.
