The Mental Health Therapist - Staff is an integral member of the Primary Health Care Team and works in part of the network team in the medical home model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required.

Experience w/ clinical interventions in Integrated Care or Community Health Care
Experience with clinical interventions related to Primary Health Care
Experience in brief treatment modalities in PHC setting is an asset
Education
Degree in Human Services/Health Sciences or related field
Demonstrates ability in teaching, facilitation and group activities
Knowledge/skills in the principle and practices that align to behavioral health
Knowledge of principles and implementation of behavioral health skills
Knowledge of community resources
Ability to provide brief, solution focused interventions
Ability to adapt to and implement change
Knowledge of Health Information Protection Act and other legislation.
Knowledge of the Mental Health Services Act and Family Services Act
Ability to work flexible hours
Knowledge of motivational interviewing
Ability to follow structure and maintain routine
Ability to work under stress
Ability to support improved functioning.
Ability to evaluate the mind-body-behavior connection
Adherence to the Code of Ethics.
