The Mental Health Therapist - Staff is an integral member of the Primary Health Care Team and works in part of the network team in the medical home model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required.

Experience



Experience w/ clinical interventions in Integrated Care or Community Health Care Experience with clinical interventions related to Primary Health Care Experience in brief treatment modalities in PHC setting is an asset

Degree in Human Services/Health Sciences or related field

Professional registration with applicable professional assoc. (if applicable) Travel within Saskatchewan Preference to HSAS members as per the SAHO/HSAS Collective Agreement Responsibility and accountability at all times for her/his own practice Outlook is located approximately 94 km south of Saskatoon Valid Class 5 driver's license
