Job Function/Purpose:
The Mental Health Outreach Worker is responsible for connecting with high risk community members to assist them to access support services, as well as coordinating suicide responses for community members of all ages, and other community crises, and arranging for follow up supports.
Duties:
It is important to note that these represent the range of potential activities that can be carried out through this position. It is necessary to prioritize which activities will actually be provided based on community needs, worker time and existing resources.
Assist with the establishment of a community-based mental health crisis management program;
Assist to enhance community management and control over crisis interventions;
Provide tools to aid in crisis intervention;
Provide educational resources;
Coordinate responses to suicide concerns, using approved intervention and assessment methods;
Ensure immediate responses to high risk crises;
Coordinate follow up services to individuals and families experiencing suicide concerns or other crises;
Coordinate follow up services to individuals, families and community after a completed suicide;
Assist community members experiencing mental health issues to access appropriate mental health services;
Coordinate training for staff and community around suicide prevention, intervention, assessment and after-care;
Provide referrals to support services for community members experiencing mental health crises (including suicide concerns);
Organize Prevention and Promotion activities, such as workshops;
Provide mental health counselling services in an effective, professional and courteous manner (when qualified);
Promote community awareness of mental health programs and services;
Liaise with other agencies/services to provide mental health services (e.g. VIHA Mental Health/Addictions, CYMH, Probation, etc.)
Advocate for community members around mental health issues with agencies and organizations such as MCFD, RCMP, Schools, Probation, etc.;
Act as a strong team member in intake, case management and crisis intervention systems;
Complete accurate and confidential records of all activities;
Adhere to all approved standards, policies and procedures;
Complete accurate and timely reports on all activities;
Ensure services are delivered in a culturally appropriate and safe manner;
To fulfill other related duties as directed.
3. Job Qualifications/Employment Requirements:
Degree, certificate, or diploma from a recognized post-secondary training program in counselling (e.g. Certification as a level II Counsellor and/or Family and Youth Counselling Certificate)
Completed training in Suicide Intervention
Valid Driver's Licence
Ability to exercise sound judgement in setting priorities and dealing with confidential and sensitive issues;
Demonstrated problem solving skills;
Excellent interpersonal skills, professionalism and integrity;
Excellent verbal and written communication skills (minimum Grade 12 English skills);
Excellent Microsoft Office computer skills.
Knowledge of mental health, substance abuse and skills in prevention
Knowledge of positive family systems and cultural practices
Experience with the Gwa'sala-'Nakwaxda'xw community and surrounding communities;
Experience with addictions prevention programs;
Three years experience in community based health/social service programs;
Experience living healthy lifestyle as a role model.
4. Working Conditions:
Week days, 30 hours per week; some evening or week-end work may be required;
Some travel may be required;
The position adheres to the personnel policies, procedures and code of ethics of the Gwa`sala-`Nakwaxda`xw Nations.
