The Adult Community Services portfolio is requiting to this exciting new opportunity which will assist in supported entry and discharge for adults into Addiction and Mental Health - Adult Community Services programs. Reporting to the Manager and under the direction of the Clinical Supervisor, the Transition Coordinator functions in the context of a multi-disciplinary team that provides quality, client-focused, recovery-based, concurrent capable, culturally appropriate care to those seeking assistance/services within Addictions and Mental Health. The Transition Coordinator is responsible for facilitating timely transitions for those clients who require supported mental health care to one of the Adult Community Services programs. The Transition Coordinator will liaise with access mental health and act on behalf of the community team to ensure a coordinated effort to ensure continuity of care. The candidate will work with the inpatient and community multidisciplinary teams to identify and address any barriers that may occur regarding entry and discharge. The Transition Coordinator provides direct follow up with clients who have recently been referred to Mental health services. Additionally, the Transition Coordinator will work with existing Adult Community Service clients to assist with connections to Primary Care and other resources within the community as needed. Additionally, the transitions coordinator is the key point person for clients attending the Orientation to therapy groups. As a Mental Health Clinician, you will work as a member of the multidisciplinary health care team collaborating on the delivery of care for the needs of individuals, families and/or communities. You will be responsible for the intake, clinical interviews and screening of clients with addiction and mental health disorders. You will liaise with a variety of internal and external stakeholders and work collaboratively with other team members as required. You may have the opportunity to participate in program planning, development and to provide supervision to practicum students. This position is open to Registered Nurses, Registered Psychiatric Nurses, Registered Social Workers, Registered Psychologists, and Occupational Therapists.
