ABOUT THE ROLE:





As a part of the interprofessional primary care team, the Case Manager's role is to support the development of a comprehensive supportive treatment plan, coordinate physical, emotional, mental, spiritual, and social health services, and to make appropriate referrals and linkages to community services and resources. The Case Manager provides services in a supportive, confidential, and culturally safer manner to help clients work towards their goals and access the resources they need to fully participate in their health care services.

RESPONSIBILITIES:




Maintain a time-limited and goals focused case load from internal referrals sent by Mamaway Wiidokdaadwin clinicians and team members Screen clients for eligibility in case management through MWIIPCT Perform a comprehensive assessment of a client's social and community needs as they relate to a case management service Develop rapport, trust and ethical relationships with clients and stakeholders Comply with all relevant standards, policies, and legislation Demonstrate respect for client's individuality and autonomy Performs other duties as assigned.



QUALIFICATIONS:




Completion of a Social Service Worker Program and 2 years experience; or an equivalent combination of education and experience. Completion of training in case management considered an asset Completion of a Social Service Worker Program and 2 years experience; or an equivalent combination of education and experience. Completion of training in case management considered an asset Demonstrated knowledge of health, social and community-based resources required to provide case management services Communication skills to explain the case management process to clients and collaborators including the processes of screening, assessing, planning, implementing, evaluating, and transitioning.
