Position summary
UHN is seeking a Manager of Professional Practice (MPP) for the Social Medicine & Community Collaboration Program within the Medical VP Collaborative Academic Practice Portfolio. This role co-designs, implements, and evaluates models of care that improve access, continuity, and outcomes for underserved and structurally marginalized populations across hospital, primary care, and community settings.
The MPP collaborates with the Family Health Team, Mental Health and Addictions Program, Emergency Departments, and community partners such as IMPACT and Inner-City Health Associates (ICHA) to align care pathways, strengthen transitions, and reduce system fragmentation.
The successful candidate will work with interprofessional teams to build shared purpose, cohesive practice, and psychologically safe learning environments. The MPP supports capacity building, quality improvement, and change adoption through collaborative problem-solving and collective accountability.
Reporting to the Director of Professional Practice, the MPP advances evidence-informed practice standards and care integration in partnership with staff, leaders, patients, families, and community collaborators. The work is grounded in trauma-informed, culturally safe, and harm-reduction-oriented care aligned with UHN's Purpose, Values, and Principles.
