Your Opportunity:The Care Manager will provide daily direct inspired leadership and clinical oversight to the Adult Community Services (ACS) Team at the 108 Street AMH clinic. Applying the best evidence informed practice and using a continuous quality improvement approach the Manager will ensure that services delivered by the team(s) meet the needs of service users and stakeholders. The Care Manager will provide coaching and assist in coordinating the delivery of competency-based education related to the Addiction and Mental Health Principles of Care and the delivery of three streams of service, mental health, addictions and concurrent disorders. As the Care Manager, you will have the accountability to ensure that the multidisciplinary team on site delivers effective, efficient and high-quality services to individuals presenting with addiction and mental health concerns.Description:As Addiction and Mental Health Services are provided along a continuum, it is imperative that the Care Manager establishes and maintains excellent working relationships with other program areas that clients may come in contact with, in order to facilitate seamless and continuous care. The Care Manager is also the key liaison for Community Agencies, Adult Acute In-patients, physicians, Access 24/7 and other community-based Addiction and Mental Health Programs. He/she will also use information from many sources to monitor the efficiency and effectiveness of how resources are being utilized and will make changes within his/her scope to improve. While not responsible for the overall budget of the program, the Care Manager is expected to have an awareness of expenditures and cost drivers that have an impact on the budget and to communicate this to the Program Manager. The Care Manager will have the signing authority required to facilitate daily operations. The Care Manager will provide coaching and assist in coordinating the delivery of competency-based education related to the Addiction and Mental Health Principles of Care and the delivery of three streams of service, mental health, addictions and concurrent disorders. The Care Manager will have the opportunity to recruit highly skilled and committed staff as well as oversee the daily operations of the Team. A working knowledge of the Collective Agreements (HSAA, AUPE GSS, and UNA) is required, as is an ability to deal effectively with performance issues. The Manager will also be responsible for scheduling employees, approving vacation, and reinforcing the importance of staff attendance, reliability and punctuality.
