The Care Manager will provide direct supervisory support to Nurses, Social Workers, and Administrative Support staff working at the Southern Alberta Clinic (SAC) along with the Primary Health Care Collaborative Program (PHCC). The Care Manager works to provide clinical support, supervision and direction to staff in under the Calgary & Area portfolio. Developing and maintaining strong working relationships with key stakeholders in collaboration with the Medical Director of SAC, and acute care sites leadership & physicians is crucial. The PHCC is a community/acute care program and focuses on complex patients who have challenges with overcoming their biopsychosocial barriers and accessing health care. The staff work in an intensive case management framework to provide care and support through discharge care planning in partnership with acute care units and primary care physicians. The position is responsible and accountable for making program level decisions and having direct leadership responsibility for contributing to the achievement of the goals of SAC & the PHCC. As the Care Manager you will need to have excellent skills with healthcare computer programs and data management. You will have the opportunity to optimize the clinic's integration in EPIC/Connect Care. You must demonstrate a commitment to patient satisfaction and quality health services; have strong communication, professionalism, facilitation and training skills; demonstrate critical thinking, time management, conflict resolution, decision-making and problem-solving skills; be adaptable in an environment with frequent interruptions and short timelines while fostering collaboration with members of the interdisciplinary team. As well, you will be responsible for financial accountability for the operating budget as well as overseeing a fund provided from the Calgary Health Foundation.

The Care Manager for the Southern Alberta Clinic is responsible and accountable for the overall operational leadership of the clinic, and its programs, which provide multidisciplinary care to patients who live with chronic conditions that may require complex health management. This includes fostering a culture of high quality, collaborative care, continuous improvement and the effective deployment of resources. As the Care Manager, it is required to actively participate in budget planning, resource allocation, workforce optimization and performance development. This position requires exceptional skills in change management, strategic and collaborative leadership, and the ability to engage a multidisciplinary team in providing patient-centered ambulatory care. You will also coach SAC and PHCC staff to enhance client interactions and debrief challenging situations. The Care Manager will need to communicate and work with the Northern Alberta HIV Clinics and community HIV programs. Through effective communication strategies and involvement in various initiatives, you will support the vision of the programs under the Calgary & Area portfolio.
