Manager ALC, Patient Navigation And DischargePermanent Full TimeReporting to the Program Director, the Manager of Patient Navigation & Discharge Planning is accountable for the ongoing development, implementation and continuous quality improvement in the discharge planning process across programs and services. The candidate brings extensive clinical knowledge and experience in the areas of discharge planning practices, guiding legislation, community resources and linkages, best practices supporting Alternative Level of Care (ALC patients) within the hospital and community sectors, which will serve as the foundation for providing clinical oversight, leadership and collaboration to inter-professional teams across the corporation.Contribute to safe, quality care by

  • Supports the inter-professional team in the development of service-oriented solutions that best meet patient/family care needs and cultivates effective inter-professional relationships at all levels.
  • Supports the implementation of best/leading practices, quality documentation and accountability models.
  • Leads planning and co-ordination of complex discharges or discharges of non-admitted patients within the Emergency Departments, including collaboration with Social Worker, GEM Nurse, Crisis Worker or Hospital Care Coordinator, as required.
  • Responsible to support teams and provide leadership, assuming management accountability when escalation beyond
  • Supports inpatient units in developing a discharge strategy for complex cases
  • Accountable to ensure best practices and compliance with applicable legislation by clinical team in the in management of ALC and discharge planning process.
  • Ensures timely escalation of admitted patients who don\xe2\x80\x99t have a discharge plan, following the Central Region Discharge Planning Pathway.
  • Oversees the MHelps and Mhelps Plus program to support decreasing length of stay and safe discharges
  • Expert resource for clinical team, patients and families in understanding and applying applicable legislation including but not limited to the Hospital Act, Long Term Care Act 2021, Substitute Decision Making, Landlord and Tenant Act.
  • Expert resource for Resource Matching & Referral, EPIC electronic Discharge Planning Dashboard, tableau reports and other relevant metrics.
  • Direct supervisor for the corporate ALC Patient Care Coordinator
  • Oversees the workload to ensure both sites (MHRHH and CVH) are effectively provided with on-site discharge planning expertise and support
  • Chair of Joint Discharge Planning Committee
  • Corporate Lead for ALC and acts as MH representative on all Ontario Health Regional Committees/initiatives.
  • Assumes Hospital partnership Lead with Home & Community Care Support Services.
  • Completes management reports addressing program issues and reviewing approved budgets, including strategies to match available resources with patient need.
  • Identifies issues & challenges; monitoring trends requiring targeted interventions and change in practice patterns or processes.
  • Works collaboratively with the Program Leaders and taking joint accountability for achieving performance targets, managing risk, improving quality & safety and advancing patient care outcomes, under the direction of the Program Director.
  • Coaches and mentors, facilitates and supports the creation and maintenance of an environment that fosters excellence.
  • Demonstrates relevant on-going professional development and continuing learning.
  • Leads program based and corporate initiatives to enhance efficiency and cost effectiveness that maintain or improve quality of patient care and patient experience.
  • Monitors performance and takes appropriate action to respond to performance management issues on a timely basis, according to Hospital policies and procedures and relevant legislation and liaises with Human Resources as appropriate.
What must you have?
  • Completed Baccalaureate Degree in Nursing or related
  • RN Certificate of Competence from the College of Nurses of Ontario.
  • Minimum of 3 years clinical frontline management experience
  • Minimum 5 years clinical experience in acute care.

  • Strong analytical, problem solving and critical thinking skills with an ability to understand, analyze and report on complex information in a timely manner.
  • Excellent interpersonal and relationship building experience including a proven ability to build and develop effective relationships with health care service providers and community partners.
  • Effectively uses empathy in interactions with others.

  • Advanced level proficiency in Microsoft Word, Excel, and PowerPoint.
  • Proven attendance record.
  • Ability to perform the essential duties of the job, including working regularly at both sites (Mackenzie Richmond Hill Hospital and Cortellucci Vaughan Hospital)
Preferably, your profile also includes:
  • Relevant Master\xe2\x80\x99s degree (MHA, MBA or Nursing) or evidence it is progress.
  • CNA certification in Geriatric Nursing, Palliative Nursing
