Position Overview

:
We are seeking a dedicated and skilled LTCF RAI Coordinator (LPN) to join our team. This role is essential in ensuring the accurate assessment and documentation of resident care needs in a long-term care facility. The ideal candidate will possess a strong background in nursing, particularly in long-term care settings, and will be responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring compliance with regulatory standards, and enhancing the quality of care for our residents.

Responsibilities

:

Coordinate the RAI process, including assessments, care plans, and documentation. Collaborate with interdisciplinary teams to ensure comprehensive resident evaluations. Conduct resident assessments and monitor changes in health status. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Provide training and support to staff on RAI processes and documentation standards. Participate in quality assurance activities to improve resident outcomes. Assist in developing individualized care plans based on assessment findings. Ensure compliance with state and federal regulations regarding resident assessments. Engage with residents and families to address concerns and enhance care experiences.

Qualifications

:

Current LPN license with the British Columbia College of Nurses & Midwives (BCCN&M) Minimum of three (3) years experience in senior care, preferably in a long term care setting. Experience with the use of PointClickCare. Currently LTCF RAI Course Completion.
