The Job Summary:



The LTC Occupational Therapist (OT) will provide resident care through assessment, development of individual treatment programs/consultation, direct or indirect therapeutic interventions, evaluation of treatment and discontinuation of treatment as appropriate and referral to other resources/services. The LTC OT will be ultimately responsible to the LTC Manager of Quality and Best Practice for providing quality care and maintaining departmental standards.

Key Accountabilities & Success Criteria:





Be responsible for the assessment and treatment of assigned LTC residents as appropriate and to maximize their safe and independent function. Provide education, direction and supervision to Occupational Therapy Assistants (OTA)/Physiotherapy Assistants (PTA), clinical teams, as well as to the residents and their families and the members in the community as appropriate. Maintain adequate records of assessments, treatment plans, progress notes, resident participation and other statistics, as necessary, in accordance with departmental guidelines and the OT College of Ontario. Maintain liaison with LTC multidisciplinary team, other healthcare disciplines and community agencies involved in the resident's ongoing care. Assure safe working practices and conditions through compliance with occupational health and safety regulations. To assist in the maintenance of safe equipment. Report unusual occurrences and incidents promptly to the manager. Adhere to provincial professional college guidelines of practice. Complies with resident safety programs, and processes including identifying and reporting adverse events/near misses. Attends and participates in appropriate safety training or education updates (ie. hand hygiene, emergency code reviews). Identifies unsafe conditions or practices and takes appropriate action to address them including reporting or removing hazards. To participate in team conferences, presentations, educational seminars and other departmental, health centre and community committees. Improve impaired sensory-perceptual motor function and psychosocial skills. Attain an optimum level of independence through treatment and use of adaptive device. Attain an optimum level of physical and mental function in self-care and life skills. Assess the resident's needs for an appropriate wheelchair and adaptations, provide an appropriate wheelchair and train the resident in the use of the chair. Select, fabricate and fit the resident with the appropriate adaptive device to meet the resident's needs in ADL performance. Identify the resident's limitations in Activity of Daily Living (ADL) and Restorative Care skills and train the resident in such skills. Evaluate the resident for appropriate group activity programs. Plan the resident's transfer or discharge in conjunction with other team members Formal and or informal assessments for residents with dementia to monitor changes and ensure their safety and well-being including mandatory program strategies and behavioural issues associated with dementia, such as agitation and aggression. Assessment of life skills performance and architectural barriers in the neighbourhood setting. Recommendations of necessary changes in the neighbourhood environment and facilitate safe independent functioning. Recommendations for following-up programs.


Qualifications:





Master's in Occupational Therapy and registered with the College of Occupational Therapists of Ontario (COTO). Registered Authorizer - Ministry of Health - Assistive Devices Program - Mobility Devices (including wheelchairs and positioning devices) Minimum of one (1) year of experience in a health care setting. Member in the Canadian Occupational Therapy Association (CAOT) preferred

Skills & Abilities:





Excellent time management, organizational and prioritization skills as relates to patient care. Superior communication and clinical reasoning skills. Ability to demonstrate a strong customer service philosophy. Demonstrated to be self-directed, seeking direction appropriately.
