POSITION SUMMARY:




The

Long-Term Care Informatics Specialist

plays a key leadership role in managing, developing, implementing, and overseeing all aspects of the interRAI LTCF and health outcomes for better information and care (HOBIC) projects in the Cardinal Ambrozic Houses of Providence ("the Houses"), Oversees the MDS team to support the MDS functions and submissions. These systems are critical to ensuring data accuracy and compliance with Fixing Long-Term Care Act, 2021 (FLTCA) regulations and directly influence provincial funding allocations for nursing and personal care services.


The Specialist provides expertise, education, and guidance to interprofessional teams to ensure best practices in electronic documentation, care planning, and reporting. This role supports staff education, system quality assurance, and the continuous improvement of data-driven resident care delivery in the long-term care setting.


DUTIES AND RESPONSIBILITIES:




Lead and oversee the implementation, education programs and ongoing management of interRAI LTCF
, Point Click Care and HOBIC projects and documentation at the Houses.
Develops strategies to effectively manage and track implementation, monitor outcomes and evaluate results for the interRAI LTCF
in the Houses, with the goal of accuracy and integrity in outcomes for continuity and appropriateness in provincial "levels of care" funding allocations. Conduct audits and quality reviews to ensure data integrity and adherence to MLTC and LTCF requirements. Collecting, analyzing and disseminating quality data to the team to promote continuous quality improvement, maintaining accountability for the quality and integrity of data reported to external agencies, e.g., CIHI, Ministry, including submissions and sign off;
Collaborate with managers, registered staff, and interprofessional teams to ensure proper documentation and care planning standards are met. Act as a resource for clinical and administrative staff, providing guidance and support on informatics-related workflows. Incorporate 'best practice' initiatives into all interRAI LTCF
and HOBIC/PCC electronic documentation processes.
Manages and creates reports to support QI and learning initiatives - i.e. data collection and reports for Surge learning, QIP; Championing best practices and plays a leadership role in quality and process improvement initiatives in support of excellent resident care. Supporting with the orientation of new clinical and other inter-disciplinary staff (RNs, RPNs, PSWs, Activation Assistants, etc.) Demonstrate and promote the Mission, Vision, and Values of Providence Healthcare in all aspects of work.

QUALIFICATIONS/SKILLS REQUIRED:

+ Bachelor of Science in Nursing (BScN) required. + Current Certificate of Registration as a

Registered Nurse (RN)

with the

College of Nurses of Ontario

.
+ Minimum of five (5) years of clinical bedside nursing experience, preferably in long-term care or geriatric settings.
+ Experience in a leadership role within a healthcare environment.
+ Demonstrated proficiency with electronic documentation and care planning systems;

Point Click Care

and

LTCF

experience preferred.
+ Knowledge of LTCF and HOBIC systems, including compliance and reporting standards with CIHI and MLTC.
+ Understanding of adult education principles, experience developing and delivering training programs is an asset.
+ Strong analytical, critical thinking and organizational skills.
+ Excellent interpersonal, coaching, and communication abilities.
+ Commitment to confidentiality, quality improvement, and resident-centered care.
