Job Description

A Support coordinator position with office hours Monday to Friday for those nurses wanting more of a work-life balance. This role outlines a variety of services including, administrative support and initial/ongoing client and staff support.

Primary Responsibilities

Provide intake assessments with family and clients to discuss services and support plans.

Establish rapport with families and clients, while maintaining professionalism and appropriate boundaries.

Create care documents.

Provide supervision for Field Supervisors and Team Lead staff.

Maintain proper documentation.

Provide ongoing case management and follow up for client and staff satisfaction.

Assisting the Support Coordinators with delegated tasks.

Demonstrate the ability to adapt to dynamic roles of a team in the office setting.

Education/Experience Requirements



Licensed Practical Nurse Diploma

Active registration with CLPNA

Current Criminal Record check

Current CPR - BLS HCL

Experience in home care, health care, senior-related industry, and supports for people with disabilities is preferable.

Must possess a valid driver's license and vehicle

Knowledge, Skills and Abilities



Dedicated to learning and growing within the team setting and WRC policies and procedures.

Ability to work independently and with team members in an office setting and in-home setting.

Administrative experience is an asset.

Maintain client and family confidentiality, in accordance with office policies.

Provide appropriate judgement and decision-making skills as required and ongoing.

Ability to adapt to changes and critical situations as required.

Provide appropriate reporting, collaboration, and communication with direct supervisor and team members.

Demonstrate excellent written and verbal skills.

Excellent organization and ability to prioritize tasks and meet deadlines.

Provide knowledge and guidance to Field Supervisors and Field staff.

Ability to work with complex clients and families with specific needs.

Able to build rapport and establish trust.

Present in professional appearance and manner.

Able to adjust working hours if required.

Experience using computer programs and software ie: Microsoft Office suite, other reporting software systems.

Job Types: Full-time, Part-time

Pay: $24.00-$26.00 per hour

Expected hours: 30 per week
