What You'll Do:

As a Nurse Case Manager, you'll be the key professional coordinating care that is safe, effective, and empowering.

Develop and oversee personalized care plans in collaboration with clients, families, and healthcare partners. Manage cases from intake through discharge, including assessments and follow-ups. Mentor and support care staff, including performance reviews and team meetings. Provide training and communication to ensure staff are prepared for changing client needs. Support logistics such as supply management and PPE distribution. Maintain accurate, timely documentation of all client care activities.

What You Bring:

Current

LPN license with BCCNM (no restrictions)

Experience in community health (asset but not required--mentorship provided) Commitment to high-quality, client-centered care Valid driver's license and clear criminal background check
