Job Overview


The Home Care Nurse plays a vital role in providing high-quality nursing care to patients in their homes. This position requires a compassionate and skilled professional who can assess patient needs, implement care plans, and collaborate with healthcare teams to ensure optimal patient outcomes. The Home Care Nurse will work with a diverse patient population, including seniors, children, and individuals with chronic illnesses or disabilities.

Duties



Conduct comprehensive patient assessments to determine care needs and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Provide personal care assistance, including bathing, dressing, and grooming. Monitor patients' health status and report any changes to the healthcare team. Assist with phlebotomy and laboratory tests as needed. Educate patients and families on health management, medication administration, and care procedures. Coordinate with multidisciplinary teams for case management and continuity of care. Maintain accurate medical records using electronic health record systems such as Epic or Cerner. Offer support in pain management and hospice care when necessary. Foster a supportive environment for patients with dementia or other cognitive impairments.

Skills



Nursing home experience is preferred for understanding long-term care dynamics. Hospital experience is valuable for acute care situations requiring immediate attention. Proficiency in acute care practices to manage complex health issues effectively. Experience in dementia care to provide specialized support for affected individuals. Background in pediatrics is beneficial for caring for younger patients. Home care experience is essential for delivering personalized nursing services in a home setting. Strong patient care skills to ensure compassionate and effective treatment. Knowledge of medical terminology to communicate effectively within the healthcare team. Expertise in medication administration to ensure safe delivery of treatments. Familiarity with medical office procedures enhances organizational efficiency. Experience with assisted living facilities can provide insight into community-based care options. Understanding of senior care practices to address the unique needs of elderly patients. Hospice care experience is important for providing end-of-life support compassionately. Laboratory experience can assist in performing necessary diagnostic tests accurately. Skills in pain management are crucial for improving patient comfort levels during treatment. Case management capabilities are essential for coordinating comprehensive patient services.
