Job Description

Townsview Estates is a 51-bed RCF (Residential Care Facility) located in Truro, NS, licensed by the Province of Nova Scotia. Position Scope

The LPN works collaboratively within the resident care team under the direction of the Director of Care, delivering and supervising resident care, administering medications, maintaining a safe and sanitary environment, and supporting programs that promote independence and leisure. All duties are performed in alignment with the Nova Scotia College of Nursing's Standards of Practice and Code of Ethics for Licensed Practical Nurses.

Responsibilities

1. Organizational



a. Demonstrates knowledge of and conducts duties in alignment with Townsview Estates' philosophy, mission, policies, and procedures.

b. Responsible for overall operation of building when senior management is not present and contacts Management in emergency situations in accordance with Townsview Estates' All Hazards Plan.

2. Provision of Care



a. Applies the nursing process to develop individualized Nursing Care Plans, involving interdisciplinary teams, residents, and families.

b. Coordinates services across departments to meet residents' holistic needs (physical, emotional, spiritual, and social) and provides effective liaison between the individual resident and these services.

c. Administers, assesses, and documents medications in accordance with physician orders and facility protocols.

d. Reviews and processes physicians' biannual assessments with attention to detail, ensuring accuracy and resolving any identified errors or omissions.

e. Consults with pharmacy to ensure resident medication needs are met and annual medications reviews are completed.

f. Receives MAR sheets from pharmacy and reviews them thoroughly for accuracy, identifying and correcting any errors or omissions as necessary.

g. Receives medications from pharmacy and ensures all packs contain the correct medications according to the name, shape and colour of the pill listed on the pack.

h. Ensures nursing procedures follow Townsview Estate's Policies & Procedures.

i. Assists care workers with AM and HS cares daily and ensures that all cares are completed.

j. Delegates tasks appropriately to care staff in consultation with Director of Care, monitors and audits care provided to residents by nursing care staff.

k. Maintains accurate medical records including care plans, progress notes, treatment logs, Risk Management reports, M.A.R. sheets, intake/output forms, blood sugar and blood pressure logs, etc.

l. Oversees delegation and execution of tasks necessary for carrying out physician orders including medication reviews, physical updates, blood work requisitions, referrals, and documentation checks.

m. Organizes paperwork and charts for, and participates in, Resident Care Planning meetings.

n. Performs admission, transfer, and discharge procedures as directed.

o. Monitors resident's condition at a frequency appropriate to the medical status of the resident and, if necessary, contacts Emergency Health Services to arrange transport to hospital for medical assessment following on-call guidelines and Townsview Estates' Policies.

p. Collects clinical data to assist physicians in diagnosis and evaluates treatment responses.

q. Assists Director of Care and Nova Scotia Health-Continuing Care in evaluating residents for reassessment as needed.

r. Refers resident to Dietician, Physiotherapy/Occupational Therapy as needed.

s. Provides direction to family members and staff regarding resident appointments, and ensures reports from those appointments are received, reviewed, and accurately documented.

t. Maintains open communication within the nursing department and across all departments.

u. Upholds confidentiality in all aspects of resident care.

3. Health and Safety



a. Maintains knowledge of and adheres to Townsview Estates Emergency Procedures, Infection Control, Occupational Health & Safety, and Risk Management Policies.

b. Demonstrates safe use and care of equipment.

c. Ensures nursing procedures comply with Townsview Estates Policies and Procedures.

4. Other Related Duties & Responsibilities

a. Maintains clinical competencies and Personal Development (PD) skills in accordance with Nova Scotia College of Nursing licensing requirements.

b. Promotes staff education by identifying areas for improvement and reporting to the Director of Care.

c. Orientates new LPNs and ensures RCA/CCA staff follow the orientation program.

d. Participates in Townsview Estates committees, staff meetings, in-service programs, and workshop.

e. Completes staff incident reports and administers First Aid as needed.

f. Maintains effective intra- and interdepartmental relationships.

g. Completes annual core competencies (WHMIS, Fire Safety, Policy Review).

h. Completes assigned online education.

i. Adheres to the Declaration of Confidentiality.

j. Works flexible hours including days, evenings, nights, weekends, holidays, and overtime as required.

k. May be called back during emergencies under the CAREQ plan.

l. Follows Standard Operating Procedures and Safe Work Practices.

m. Actively participates in workplace safety and environmental programs.

n. Performs other related duties as assigned.

Qualifications and Skills



1.

Current registration with Nova Scotia College of Nursing (NSCN)

2.

Valid First Aid/CPR Certification

3.

Current WHMIS, Fire Safety and OHS training.

4.

Ongoing education in gerontology, Dementia care, and other relevant Long-Term Care topics.

5.

Demonstrated theoretical knowledge and practical skills in Long-Term Care.

6.

Commitment to continuous professional development.

7.

Experience in Long-Term Care (preferred).

8.

Certification in blood specimen collection or willingness to obtain certification.

9.

Strong leadership, interpersonal, supervisory, and problem-solving skills.

10.

Effective written and verbal communication.

11.

Mentally and physically capable of performing assigned duties.

12.

Maintains required certifications per established schedule

Job Types: Part-time, Fixed term contract

Pay: $36.03 per hour
