We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in various EMR/EHR systems. This role offers an opportunity to provide high-quality patient care across multiple settings, including acute care, long-term care, and community health environments. The LPN will play a vital role in patient assessment, medication administration, and supporting interdisciplinary teams to ensure optimal health outcomes.
Duties
Conduct comprehensive patient assessments, including vital signs, physical examinations, and history taking.
Administer medications and injections accurately following physician orders and protocols.
Perform IV infusions, catheterizations, and sterile procedures with adherence to aseptic techniques.
Manage airway care, ventilator support, and tube feeding as required.
Document all patient interactions thoroughly using EMR/EHR systems such as Epic, Cerner, or Athenahealth.
Assist with diagnostic evaluations including laboratory specimen collection, processing, and medical imaging procedures.
Provide wound care, suturing assistance, and post-operative monitoring.
Support patients with complex needs such as dementia care, Alzheimer's care, hospice care, and behavioral health management.
Educate patients and families on health coaching, medication management, discharge planning, and chronic disease management.
Collaborate with multidisciplinary teams in settings like ICU, emergency room, outpatient clinics, nursing homes, assisted living facilities, or home & community care environments.
Ensure compliance with HIPAA regulations and infection control standards to maintain patient confidentiality and safety.
Qualifications
Valid LPN license in the state of practice.
Proven experience in diverse clinical settings such as ICU (Level I & II Trauma Centers), emergency medicine (ER/urgent care), medical-surgical units, geriatrics (including memory care), pediatrics (including neonatal ventilator), or home & community health.
Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and pharmacology related to medication administration and pain management.
Experience with specialized procedures such as phlebotomy, venipuncture, dialysis access management (catheterization), spinal taps, gastric lavage or dermal fillers is a plus.
Ability to work independently while collaborating effectively within interdisciplinary teams.
Excellent patient assessment skills with a focus on trauma response (Level I & II Trauma Center experience), infectious disease care or acute pain management preferred.
Knowledge of HIPAA regulations and infection control practices essential for maintaining compliance standards.
Flexibility to work various shifts including inpatient or outpatient settings; travel nursing experience is advantageous for some roles.
