The Licensed Practical Nurse (LPN) - Admissions & Care Coordinator supports new residents through a smooth transition, promotes independence, and oversees ongoing health and wellness. Responsibilities include conducting pre-admission assessments, developing and updating individualized care plans, coordinating Health Care Aide (HCA) services, and acting as the main point of contact for residents and families.
Key Responsibilities
Complete health assessments prior to move-in and identify care needs.
Develop, implement, and update person-centered care plans.
Coordinate and delegate care to HCAs; connect residents with specialized services when needed.
Provide education and support to residents and families on health, wellness, and community resources.
Maintain accurate documentation in line with professional and regulatory standards.
Collaborate with the care team to promote overall resident well-being.
Qualifications
Licensed Practical Nurse (LPN) in good standing with CLPNA
Experience in geriatrics, community health, or independent living preferred.
Strong skills in assessment, care planning, and coordination.
Excellent communication, organization, and teamwork abilities.
Additional Requirements
Criminal Record Check, TB Test (within first month), First Aid with CPR, WHMIS Certification.
