PART TIME LPN POSITION 3PM TO 8PM WEEKDAYS (25 HOURS PER WEEK)
To provide optimal healthcare and nursing services, within the LPN scope of practice for the Guests of Pihtikwe Facilities and to Alexander Band members and under the direction of the Health Director and supervision of the LPN will perform the following duties.
POSITION DUTIES:
1.0 Care of Guests
Advocates for all Guests in all aspects of physical, emotional, mental, and spiritual well being
Implement care and treatment for all Guests' according to their nursing care plan and physicians' recommendations
Provide "hands on" or direct care for all Guests
Participates in the development of individual resident care plans & kardex
Monitors and reports changes in health status of the Guests to the RN Manager
Participates, contributes, and collaborates with the interdisciplinary team (Physician, LPN, OT, PT, Mental Health Therapist, Dietician, and other team members)
Completes documentation and communication, in a confidential manner, according to Alexander Health Services documentation policy and procedures & CLPNA
Implements nursing procedures as outlined in the care plan & kardex within the LPN scope of practice (Foot care, catheterizations, specimen collections, vital signs, dressing changes, wound care under the assessment of the Nurse Manager.
Implements resident activities of daily living (ADL), personal care, toileting, ambulation or grooming of Guests
Participates in any resident conferences, case reviews, or interdisciplinary team conferences
Monitors the nutritional status and needs of Guests and communicates any changes or concerns to the Nurse Manager.
Helps Facilitate Referrals to Rehab services
Stays in constant communication with RN in Charge or Coverage RN when new admissions are assessed for facility appropriateness.
Educate the guests on Infection Prevention and Control.
Must be available on-site when new guest arrives to properly conduct inventory and search and medication documentation.
1.1 Medication Management
Provides medication management, medication reviews and ensures the monthly Medication Administration Records (MARS) are complete and accurate
Adheres to Alexander Health Services medication management policies and procedures including management of High-Risk Medications
Administers daily medication to each resident and documents accordingly
Participates in medication management chart audits when requested by Nurse Manager
Collaborates with Champlain pharmacy or any other pharmacy to ensure resident has their necessary medications bubble packed when required for HCA observation
Maintains current knowledge of medications purpose, actions, side effect and refers to the E-CPS for resource or pharmacist
Manages the order processing of all medications, as per Alexander Health Services medication management processes (Clinical Procedure Manual)
Adheres to Alexander Health Services policies and procedures of Disclosure and Incident reporting related to medication incidents
Ensure the Medication Administration Records (MARs) and Medication Profiles (MAP) are current and on the resident's chart
Facilitates in physician and or hospital discharge medication orders and mediation processing to the Nurse Manager, and/or physician
1.2 Resident Safety
Assess each resident's safety status including the physical environment and make recommendations to the Nurse Manager or the Operations Manager
Implements immediate interventions, when necessary to ensure resident safety is maintained
Participates in Alexander Health Services safety exercises including Fire Drills and mock Emergency Response drills
Carries a safety whistle at all times while on duty
Is knowledgeable of the colored Emergency Response codes
Completes promptly and accurately Incident Reports and follows Incident Reporting processes
Ensures the AED checklist is completed daily
Ensures the bathroom is clean according to Infection Prevention Control Protocols
Provides one on one teaching for the resident and/or family when required or requested
Utilizes resources and teaching tools
Provides support and encouragement for the guests/guardian/family
1.3 Community Member Services
Provides nursing services, within the LPN scope of practice, for any Alexander Band member who comes to the facility (ex. Vital signs, dressing changes)
Participates in the annual Influenza vaccination campaign
Reports any communicable disease manifested or discovered to the Nurse Manager i.e., (Bed bugs, lice)
Participates in community events such as the annual Health Fair, Harvest Fair, or Open House
1.4 Documentation
Adheres to Alexander Health Services Documentation policies and procedures
Documents promptly on all Guests' charts using approve nursing documentation method
Communicates guest health information within the interdisciplinary team to promote quality resident focused care
Reports any serious risk concerns immediately to the Nurse Manager or physician
Ensures month end statistics are completed and given to Nurse Manager
Understands and participates in the Accreditation processes
Participates in any Quality Assurance or Accreditation assigned by the Accreditation Coordinator
Respects all Alexander Health Services privacy & confidentiality policies & procedures
Participates in any chart audit exercises requested by Nurse Manager
OTHER
Assist, cook and clean as may be deemed necessary.
Hours of work may vary from time to time.
May be required to perform duties and responsibilities in other facilities, i.e. Health Centre, Footprints Healing Centre, Headstart, school etc.
Education of Support Staff re: MAP, Hand Hygiene, SPRAM, STIBBI Prevention.
QUALIFICATIONS
Maintains current CLPNA license
Maintains current CPR, First Aide, WHMIS and TDG certificates
Maintains required vaccinations and receives the annual Influenza vaccine
Possess a valid driver's license and working vehicle and have proper passenger insurance.
Submits a current Crim and Child Security Check
Completes any task or activity requested by the Health Director
Attends all staff meetings, case conferences
Must be able to communicate effectively in both oral and written English.
Must possess excellent interpersonal skills to function with staff or the employer(s).
Must be alcohol and addiction free
Must have knowledge of FOIP, PIPA and H.I.A
THIS ROLE IS FOR A FULL TIME POSITION ON THE RESERVE AT ALEXANDER FIRST NATIONS, IT IS AT OUR HARM PREVENTION CENTER AND IS FOR EVENING SHIFT ONLY. PLEASE DO NOT APPLY UNLESS YOU ALREADY HAVE A LICENSE IN ALBERTA AND ARE ABLE TO MAKE THE COMMITMENT TO WORK ON THE RESERVE.
