We are seeking a dedicated and organized LPN Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest level of care and support throughout their home care journey. This position requires strong communication skills, attention to detail, and a solid understanding of seniors care at home. The Care Coordinator will work closely with caregivers, clients, and administrative staff to facilitate efficient care delivery. This position will be Monday to Friday dayshift, with the requirement to do on call once per month.
Responsibilities
Serve as the primary point of contact for client addressing inquiries and providing support throughout their care process.
Coordinate client assessment, ensuring timely scheduling of care staff and follow-up.
Maintain accurate client files and documentation in compliance with healthcare regulations.
Collaborate with caregivers to ensure continuity of care and effective communication among all parties involved.
Utilize assessment information to assist in the interpretation of clients needs and care plans.
Provide administrative support including managing correspondence, filing, and other office duties as needed.
Assist in the onboarding of new clients by explaining procedures and answering questions regarding their care.
Support care staff by assisting with staff training or assessment of skills.
Requirements
Previous experience in home care is required.
Active LPN registration required.
Experience with supervision and staff development preferred.
Familiarity with medical terminology is essential for effective communication within the healthcare environment.
Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
Excellent interpersonal skills to foster positive relationships with clients and team members.
Proficiency in office software applications and administrative support functions.
