JOB SUMMARY:



Under the direction of the Nursing Care Supervisor, the Licensed Practical Nurse (LPN) provides nursing care, support and services within the established scope of Assisted Living services. The LPN works with the Private Pay (PP) tenants and Health Care Assistants (HCA) to ensure the established support plans are implemented and changed as required. Carries out tasks, and liaises with professional staff and families as required. The scope of the LPN job summary extends to embrace all Menno Apartment tenants, showing compassion in their time of need. He/she is knowledgeable of and adheres to the standards of practice as outlined by the BCCNM.

The LPN is an integral member of the Menno Place team and is required to have a commitment to the vision, mission and values of Menno Place performing duties following Menno Place policies and procedures and ensuring safety for tenants and staff.

This position requires teamwork, communication and cooperation. It is the expectation that the individual in this position contributes to a positive, safe working environment, takes instruction well and cooperates with their manager and co-workers. This person manages their time well and participates in continuous quality improvement of services provided.

DUTIES & FUNCTIONS:



1. Collaborates with Life Enrichment Coordinator for the intake/screening of prospective tenants suitability for PP and to ensure tenants remain within the parameters of the program.

2. Initiates, implements, updates and evaluates the care plans for PP tenants. Promotes tenant involvement in health care decision: provides information and opportunity to discuss concerns and respects personal choices.

3. Reviews and obtains pertinent health status information, and updates information in Senior Care.

4. Responsible for PP AL record keeping and charged accordingly.

5. Reports concerns related to tenant safety to families and to the Life Enrichment Coordinator/Manager of AL Care.

6. Acts as a liaison between the tenant and Home Health, Pharmacy, families, mental health and acute care agencies.

7. Creates and reviews daily tasks sheets for related staff, evaluates services provided and implements changes to services as needed.

8. Performs nursing related duties for tenants and various treatments/procedures as outlined in the competencies required of the LPN as required.

9. Collaborates with the AL LPN Lead to ensure services for tenants are provided.

10. Attends staff in-service and continuing education programs, and approved work-related conferences and seminars.

11. Participates with on-call program for the Apartments as designated.

12. Performs other related duties as assigned.

QUALIFICATIONS:



EDUCATION, TRAINING AND EXPERIENCE:



Graduation from an approved Practical Nurse Program and holds current registration with the BC College of Licensed Practical Nurses Minimum of 2 years LPN experience A working knowledge of MSWord, Office, Excel., Senior Care and computer software programs knowledge an asset Knowledge of PP service an asset

Participates in fire safety demonstrations and fire drills and knows the organization's fire and disaster plan.
