Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patients' vital signs and observe changes in health status to report promptly to healthcare professionals
Administer medications accurately following prescribed care plans and HIPAA privacy regulations
Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper food safety standards
Operate mobility aids such as Hoyer lifts or wheelchairs safely during transfers and repositioning
Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with healthcare regulations
Support clients with cognitive challenges by implementing memory care strategies for conditions like Alzheimer's disease and dementia
Perform light housekeeping tasks including cleaning living areas to promote a safe and hygienic environment
Collaborate with healthcare teams to develop and follow personalized care plans aligned with each client's needs
Engage in patient observation to identify potential health concerns or behavioral changes requiring intervention
Provide emotional support and companionship to foster a positive environment for clients
Assist with community integration activities or outings as appropriate for the client's mobility and health status
Qualifications
Proven experience in home health care, assisted living, or long-term care environments; group home experience is a plus
Knowledge of patient monitoring techniques, vital signs measurement, and basic nursing procedures
Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred
Familiarity with dementia care, Alzheimer's care, hospice support, autism spectrum disorder support, and working with individuals with developmental disabilities or disabilities in general
Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
Strong communication skills to effectively coordinate with healthcare providers, families, and clients
Compassionate attitude with a passion for caregiving and improving clients' quality of life
Flexibility to work various shifts including evenings, weekends, or overnight hours as needed
