Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
Monitor patients' vital signs such as blood pressure, pulse, respiration, and temperature to ensure health stability.
Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations for patient privacy.
Support clients with meal preparation and nutritional needs, ensuring dietary restrictions are followed precisely.
Provide companionship and emotional support to promote mental well-being, especially for those with dementia or Alzheimer's disease.
Utilize specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges.
Conduct patient observation and report any changes in condition or behavior promptly to healthcare professionals or family members.
Maintain cleanliness and hygiene by performing light housekeeping duties related to client care environments.
Follow detailed care plans tailored to each individual's needs, including assisted living or hospice care requirements.
Support clients in community settings or within their homes by working closely with families and healthcare teams to ensure comprehensive care delivery.
Document all care activities accurately while respecting patient confidentiality and HIPAA standards.
Engage in ongoing training related to CPR, first aid, dementia care, autism support, or other specialized areas as needed for continuous improvement.
Qualifications
Proven experience in home health care, assisted living facilities, nursing homes, group homes, or long-term care environments.
Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and safety protocols such as CPR and first aid certification.
Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer's and memory care), or hospice needs.
Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina is essential.
Excellent communication skills for collaborating with clients' families and healthcare teams while maintaining confidentiality per HIPAA guidelines.
Prior experience working in home & community care settings is highly desirable.
Compassionate attitude combined with patience and adaptability when caring for diverse populations.
Valid driver's license may be required depending on the role's travel expectations; background checks are standard for employment compliance.
