Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patients' health status by observing vital signs such as blood pressure, pulse, and respiration; report any concerns promptly
Provide medication reminders and assist with medication administration following care plans and HIPAA regulations
Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary
Support clients with mobility using equipment such as Hoyer lifts or other assistive devices to ensure safe transfers
Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
Document care activities accurately in compliance with privacy standards like HIPAA and follow prescribed care plans
Engage in patient observation to identify changes in condition or behavior, especially for clients with dementia or Alzheimer's disease
Support clients in community outings or social activities to promote engagement and socialization
Collaborate with healthcare professionals, family members, and social workers to ensure comprehensive care delivery
Adhere to safety protocols during heavy lifting or physically demanding tasks while prioritizing client safety at all times
Qualifications
Prior experience in home health care, assisted living, group homes, nursing homes, or long-term care environments is highly preferred
Certification in CPR and First Aid required; additional certifications such as HIPAA training are advantageous
Demonstrated ability to work effectively with individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer's) or memory care needs
Experience working with people who have disabilities or require hospice or palliative care support is a plus
Strong observation skills for patient monitoring and recognizing changes in condition or behavior
Ability to perform heavy lifting safely when assisting with transfers or mobility support
Excellent communication skills to collaborate effectively with clients' families and healthcare teams
Compassionate attitude combined with patience and respect for client dignity
Valid driver's license and reliable transportation preferred for community outings or client visits
