Position Title: Life Skills Facilitator / Service Navigator

Reports To: Program Manager

Position Summary

The Life Skills Facilitator / Service Navigator will provide individualized and group-based life skills programming and support clients in accessing essential community services. This position plays a critical role in bridging clinical care with practical, real-world skill application, supporting clients' recovery, independence, and reintegration.

Key Responsibilities

Assess client strengths, needs, and barriers to independent living. Develop and implement life skills programs focusing on daily living, interpersonal relationships, and community engagement. Support clients in navigating social, health, and community services (housing, financial aid, employment programs, etc.). Collaborate with clinical and peer support teams to ensure integrated service delivery. Maintain detailed client records, progress notes, and outcome reports. Facilitate workshops or group sessions on topics such as stress management, goal setting, and healthy routines, etc. Advocate for clients within community systems to ensure access to required supports. Participate in case conferences, team meetings, and program evaluations.

Qualifications

Diploma or degree in Social Work, Community Development, Psychology, or related field.

Minimum two years of experience working with individuals experiencing mental health challenges.

Strong understanding of community resources, social systems, and recovery-oriented practice.

Excellent interpersonal, organizational, and communication skills.

Ability to work collaboratively in a multidisciplinary environment.

Valid driver's license and access to a vehicle preferred.

Pay: $53,000.00-$54,000.00 per year
