Job Summary


We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients in various settings, ensuring their comfort and well-being. This role requires strong clinical skills, the ability to work collaboratively with healthcare professionals, and a commitment to delivering high-quality patient care.

Duties



Administer medications and treatments as prescribed by physicians. Monitor patient vital signs and report any significant changes to the healthcare team. Assist in patient assessments and develop individualized care plans. Provide personal care services, including bathing, dressing, and grooming assistance. Perform wound care, catheterization, IV infusion, and other nursing procedures as needed. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Educate patients and families on health management and disease prevention strategies. Support patients with developmental disabilities, Alzheimer's care, memory care, and geriatrics. Participate in discharge planning and case management for continuity of care.

Qualifications



Current LPN license in good standing. Experience in critical care, long-term care, assisted living, or home care is preferred. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is a plus. Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is beneficial. Proficient in performing phlebotomy, venipuncture, and administering injections. Strong understanding of anatomy and physiology; experience with acute pain management is desirable. Ability to work effectively in high-pressure environments such as emergency medicine or trauma centers (Level I/II). Experience in pediatrics or working with children is an advantage.
