Job Summary


We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in a variety of settings, including nursing homes, hospitals, and home care. The LPN will provide essential patient care, administer medications, and support the overall health and well-being of patients across different age groups, including pediatrics and seniors. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality care.

Responsibilities



Provide direct patient care under the supervision of registered nurses and physicians. Administer medications and monitor patients for adverse reactions. Assist with daily living activities for patients in various settings such as nursing homes, assisted living facilities, and home care environments. Conduct assessments and document patient information accurately using medical terminology. Collaborate with healthcare teams to develop and implement individualized care plans. Perform phlebotomy and laboratory tests as needed. Maintain accurate records in electronic health systems such as Epic or Cerner. Educate patients and families on health management, medication administration, and pain management strategies. Support case management efforts by coordinating with other healthcare professionals.

Requirements



Valid LPN license in the state of practice. Nursing home experience preferred; hospital experience is a plus. Familiarity with acute care, dementia care, pediatrics, hospice care, and senior care practices. Proficiency in medication administration and understanding of medical terminology. Experience with electronic health records systems (Epic or Cerner) is desirable. Strong interpersonal skills to effectively communicate with patients, families, and team members. Ability to work collaboratively within a multidisciplinary team environment. Previous experience in home care or assisted living settings is advantageous. Knowledge of laboratory procedures and pain management techniques is beneficial. Pay: $25.55-$43.27 per hour
