- Provide direct patient care and support to individuals in various healthcare settings, including hospice care, medical offices, nursing homes, and pediatric facilities.
- Administer medications and treatments as prescribed by healthcare professionals.
- Monitor and record patient vital signs and medical information.
- Assist with case management and coordination of patient care.
- Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans.
- Educate patients and their families on healthcare procedures, medications, and self-care techniques.
- Adhere to all relevant legal, ethical, and professional standards in nursing practice.
SKILLS
- Experience in providing hospice care, medical office support, case management, nursing home care, or pediatric nursing.
- Proficient in caring for patients of all ages, including experience with children.
- Familiarity with electronic health record systems such as Epic and Cerner.
- Strong knowledge of medical terminology and ability to accurately document patient information.
- Ability to work in fast-paced environments and prioritize tasks effectively.
- Excellent communication skills to interact with patients, families, and healthcare professionals.
- Compassionate and empathetic approach to patient care.
- Attention to detail and strong organizational skills.
QUALIFICATION
- Graduate of an accredited school of Licensed Practical Nurses with pharmacology.
