Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This role involves supervising care plan development, conducting in-home assessments, and providing clinical support to our care team. The ideal candidate will possess a strong background in patient care, with experience in various settings such as nursing homes, hospitals, and home care. As an LPN, you will play a vital role in providing quality care to patients, assisting with their daily needs, and ensuring their comfort and safety.

Key Responsibilities:

Develop and update individualized care plans in collaboration with clients and families. Perform in-home assessments and identify health and safety risks. Provide guidance and support to care aides to ensure quality care delivery. Maintain accurate documentation and ensure compliance with BC health standards. Communicate effectively with clients, families, and management.

Qualifications

Current LPN license in British Columbia. Experience in home care, geriatric care, or community health preferred. Strong assessment and documentation skills. Excellent communication and interpersonal abilities. Valid driver's license and reliable transportation. Compassionate demeanor with a commitment to providing high-quality patient care. Pay: $28.00-$38.00 per hour
