Overview


We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide compassionate patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role involves administering medications, monitoring patient health, and supporting treatment plans to ensure the highest quality of care. Candidates with experience in acute care, dementia care, pediatrics, hospice, or senior care are highly valued. A strong understanding of medical terminology, case management, and familiarity with electronic health records such as Epic and Cerner is preferred. Responsibilities



Administer medications and treatments accurately according to physician orders Monitor and document patient vital signs and health status Assist with patient assessments and develop individualized care plans Provide wound care, phlebotomy, and pain management as needed Support patients with activities of daily living and ensure comfort and safety Collaborate with interdisciplinary teams to coordinate patient care Maintain detailed documentation using electronic health record systems such as Epic and Cerner Educate patients and families about health conditions and post-discharge care instructions Ensure compliance with healthcare regulations and safety protocols Participate in case management activities to optimize patient outcomes

Skills



Proven experience in nursing home, hospital, or home care settings Knowledge of acute care, dementia, pediatrics, hospice, or senior care is advantageous Proficiency in medical terminology and medication administration Experience with electronic health records such as Epic or Cerner Strong patient care skills with compassion and attention to detail Ability to perform laboratory procedures like phlebotomy when necessary Excellent communication skills for effective collaboration with healthcare teams and families Ability to handle multiple priorities in a fast-paced environment Familiarity with pain management techniques and case management strategies Experience working with diverse populations including children and elderly patients

Job Type: Full-time

Pay: $28.00-$46.21 per hour
