About Us

CasaHealth / HealthLink Outreach works as multidisciplinary health care team focused on providing home care and medication management in the comfort of the patient's home

The Role:

LPNs support clients and their families to achieve independence while maintaining optimal health. You will be responsible for visiting patients in their own homes to provide personal care in a one-on-one setting.

Key Responsibilities:

Picking up and dropping off medication(s) Providing nursing supplies Assisting with medication intake and administering medication (including, but not limited to: oral, topical {creams, patches, drops}, subcutaneous, intramuscular) Witnessing medication intake Monitoring all necessary vitals Documenting all medical interventions Formulating, adjusting and following up with care plans Acknowledging and recognizing patient concerns and providing appropriate follow up Communication with team via assigned format (electronic, etc.) Various shifts/routes, as assigned/scheduled

Requirements:

Good Standing With BCCNM Valid BC Class 5 Or 7 Driver's License Reliable Vehicle Business Class Insurance First Aid, WHIMS and CPR Certifications Up To Date Immunizations

Pay: $29.50-$32.55 per hour
