We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. Duties


As a Licensed Practical Nurse, your responsibilities will include:

Care Boost Pharmacy Ltd in Surrey's two locations is recruiting LPNs to provide Medication Management and Dispensing individualized care to its clients in their homes on a daily/weekly/ or as needed basis. Strong collaboration and communication with the pharmacists on individualized clients medication care; may require seasonal immunizations. Strictly follow BCCNM's scope of practice for LPNs and following all relevant standards on following client-specific orders, acting within autonomous scope, medication, documentation and other relevant standards. https://www.bccnm.ca/LPN/ScopePractice/Pages/Default.aspx Practicing and Active good standing registration with BCCNM. Administering medications and monitoring patients for adverse reactions or side effects; and educating clients on medication management. Conducting routine assessments based on nursing process such as blood pressure and blood sugar monitoring of patients' health status and reporting changes to the pharmacists and physicians. Maintaining accurate medical records and documentation using BCCNM Standards. Being proactive in addressing and communicating changes to client's health care needs and continuation of care processes such as if client admitted to hospital. Strong educational, knowledge and nursing background in gerontological nursing. Strong communication skills in English, Hindi and Punjabi. Being respective to clients Cultural, Safety, Religious, and Spiritual well-being. Daily report back to the pharmacists and other team for any changes to medications or clients health status. May include other duties as needed.

Experience


Candidates should possess:

Previous experience in nursing homes, hospitals, or acute care settings is preferred. Familiarity with medical terminology and medication administration protocols. Experience in specialized areas such as dementia care, hospice care, senior care, chronic disease management or home care is a plus. Proficiency in using electronic health records systems. A strong understanding of patient care practices and the ability to work effectively in a team-oriented environment. Pay: From $28.00 per hour
