This facility will provide a supportive and healing environment for Individuals and families struggling with substance abuse issues. Our approach emphasizes a multi-disciplinary, person-centered, and family-focused model to address the diverse needs of those seeking recovery in Saskatchewan.

The Carter House will target women and families who:

Must relinquish care of their children to access treatment.

Are seeking treatment to regain custody of their children.

Are at risk of having their children apprehended due to substance use.

Position Summary:


The Licensed Practical Nurse (LPN) at The Carter House provides comprehensive nursing support to parents and children, ensuring high-quality medical care, treatment management, guidance and medications in alignment with individualized care plans following the standards of the College of Licensed Practical Nurses of Saskatchewan (CLPNS) and facility policies. The LPN works collaboratively with Continuing Care Assistants (CCAs), Shift Supervisors, an interdisciplinary team, community health care providers, and agencies to promote holistic health and continuity of care. The role requires strong communication skills and the ability to maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds.

Key Responsibilities:



Client assessment and monitoring of overall health status utilizing the nursing process of assessment, planning implementation and evaluation.

Follow medical care plans for all parents and children residing at The Carter House.

Provide medical support, including screening, testing, treatment, and follow-up care for clients.

Preparation, administration, monitoring, and documentation of medication under RN or physician supervision.

Manage medications and ensure compliance, including Medication Administration

Collaborate with other health care professionals involved in the care of The Carter House clients.

Support interdisciplinary staff and community services to ensure full continuity of care.

Pharmacy liaison duties as required.

Assist unregulated care providers - Continuing Care Assistants (CCAs) and Shift Supervisors with care planning and coordination related to medical needs.

Supervision and delegation of tasks as appropriate, providing guidance, feedback, and evaluation.

Ensure clear and consistent communication of medical information to CCAs, Shift Supervisors, and physicians.

Actively participate with team members in the assessment, planning and evaluation of client care.

Perform toxicology screens as needed and follow up on lab results.

Support the development and maintenance of medical work standards within The Carter House.

Accompany and support clients during medical appointments when required.

Educate patients on medication adherence, relapse prevention, and healthy lifestyle habits.

Maintain accurate progress records and ensure compliance with HIPAA and safety protocols and in accordance with legal and ethical standards.

Respond to emergencies and crisis management such as overdose following the established procedures.

Orientate new workers and other duties as assigned.

Deliver client care according to the College of Licensed Practical Nurses of Saskatchewan

Qualifications:



Licensed Practical Nurse (LPN) in good standing.

Licensed Practical Nurse registered in the Province of Saskatchewan

Current basic CPR

Recent clinical experience, including medication administration and leadership duties.

Demonstrated ability to communicate and maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds.

Experience working collaboratively with healthcare teams and community agencies.

Toxicology screening, and basic clinical procedures.

Strong communication, interpersonal, organizational skills and attention to detail.

Strong problem solving and decision-making skills.

Knowledge of / experience with a trauma and violence informed care.

Demonstrated ability to work effectively as a member of the interdisciplinary team.
