Responsibilities
Provide direct patient care, including administering medications.
Collaborate with other healthcare professionals to develop and implement individualized care plans.
Responsible for witnessing ingestions and dosing medications in a care facility.
Participate in discharge planning activities, ensuring patients have appropriate follow-up care arrangements.
Familiarity with medical coding systems for accurate documentation and billing purposes.
Possible long driving hours completed in up to 30 minute intervals.
Schedule:
Rotating weekends
On-call shifts
Possible additional hours based on scheduling needs: Monday-Friday 10 AM- 4 PM.
Requirements:
Valid LPN license in Alberta, Canada.
Previous experience in a healthcare setting preferred
Knowledge of medical terminology and proficiency in medication administration.
Experience with electronic health record systems such as PCC or Telus Health, preferred but not required.
Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
Compassionate approach to patient care with a focus on enhancing quality of life.
Ability to work collaboratively in a fast-paced environment while maintaining attention to detail.
