Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in various EMR/EHR systems. This role involves providing high-quality patient care across multiple settings, including long-term care, acute care, hospice, and community health environments. The LPN will work closely with interdisciplinary teams to ensure comprehensive and personalized care, utilizing their expertise in nursing procedures, medication administration, and patient assessment. Candidates with experience in critical care, emergency medicine, geriatrics, or specialized fields such as dementia or Alzheimer's care are highly valued.

Responsibilities

Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations. Administer medications and injections accurately following prescribed protocols. Manage IV infusions, catheterizations, and sterile procedures with aseptic technique. Document patient information thoroughly using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Assist with diagnostic evaluations including laboratory specimen collection/processing and medical imaging coordination. Provide specialized care for patients with complex needs such as dementia, memory care, or behavioral health conditions. Support discharge planning and coordinate post-acute or home & community care services. Manage airway devices including ventilators and perform advanced procedures like spinal taps or gastric lavage when required. Educate patients and families on health coaching topics such as nutrition, medication management, and disease prevention. Ensure infection control standards are maintained in accordance with NCQA standards and HIPAA regulations. Collaborate with multidisciplinary teams in various settings including hospitals (Level I & II Trauma Centers), clinics, outpatient facilities, hospice programs, or pediatric units such as PICU or Neonatal ventilator units.

Requirements

Valid LPN license in the state of practice with current certification. Experience working in diverse clinical environments such as ICU, emergency room, medical-surgical units, pediatrics (including neonatal), geriatrics (including Alzheimer's and dementia care), or home & community health. Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks; familiarity with ICD-9/ICD-10 coding is preferred. Strong knowledge of medical terminology, anatomy & physiology, pharmacology related to medication administration and IV insertion. Ability to perform patient assessments accurately and efficiently while demonstrating excellent communication skills. Experience with infection control practices and aseptic techniques during sterile procedures like suturing or catheterization. Capable of working independently or as part of a team across inpatient and outpatient settings including urgent care or occupational health clinics. Ability to handle sensitive information following HIPAA guidelines and maintain confidentiality at all times. Additional skills such as teaching hospital experience, post-acute care expertise, or familiarity with specialized treatments like dermal fillers or botulinum toxin are advantageous.
