We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to adapt to diverse patient needs. The LPN will collaborate with interdisciplinary teams to ensure safe, effective, and personalized care for patients of all ages, including pediatrics, seniors, and individuals requiring hospice or dementia care.
Responsibilities
Administer medications and treatments accurately according to physician orders and established protocols
Monitor patient vital signs and assess patient conditions regularly
Document patient information accurately using electronic health record systems such as Epic and Cerner
Assist with patient admissions, discharges, and transfers while ensuring compliance with healthcare regulations
Provide wound care, phlebotomy, and laboratory specimen collection as needed
Educate patients and families on health management, medication use, and post-discharge care instructions
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
Support patients with activities of daily living in assisted living or home care settings
Maintain a safe environment by adhering to infection control protocols and safety standards
Participate in ongoing training related to pain management, case management, dementia care, hospice care, pediatric care, and other specialized areas
Requirements
Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent credential
Prior experience in nursing home settings, hospitals, acute care units, or home health preferred
Knowledge of medical terminology and medication administration procedures
Experience with electronic health records such as Epic or Cerner is highly desirable
Strong patient care skills with an emphasis on compassionate communication and attention to detail
Ability to perform laboratory procedures including phlebotomy when required
Familiarity with senior care, dementia management, hospice services, pediatrics, pain management, and case management is advantageous
Excellent organizational skills with the ability to prioritize tasks effectively
Ability to work independently as well as part of a multidisciplinary team
