Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This role requires a strong understanding of medical procedures, client needs, and effective communication to ensure optimal health outcomes. Experience in dementia care, and senior care is highly valued. The LPN will work closely with physicians, residents, and families to deliver personalized nursing services in a professional and caring manner.

Duties

Administer medications and treatments accurately following physician orders Monitor patient vital signs and assess patient conditions regularly Assist with resident admissions, discharges, and transfers Provide wound care, including dressing changes and infection control measures Document resident information accurately using electronic health records. Educate residents and families on health management, medication use, and post-discharge care plans . Care-Plans / Policy Development as per SLTC standards. Collaborate with interdisciplinary teams to develop and implement individualized care plans Ensure compliance with all healthcare regulations and safety protocols Supervise Continuing Care Assistants Work Directly with Administrator

Skills

Proven experience in nursing home, hospital, or home care settings with a focus on patient-centered care Knowledge of medical terminology, medication administration protocols, and laboratory procedures Familiarity with electronic health record systems such as Epic or Cerner is preferred Strong communication skills to effectively interact with patients of all ages--including children--and their families Ability to perform phlebotomy and manage pain or complex case management situations Experience in senior care, dementia care, hospice support, or pediatrics is advantageous Ability to work independently and as part of a multidisciplinary healthcare team Compassionate attitude with excellent organizational skills to prioritize patient needs effectively
Job Types: Part-time, Permanent

Pay: $25.65-$43.41 per hour
