Overview


We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have experience in various care settings, including nursing homes, hospitals, and home care environments. This role offers an opportunity to provide high-quality patient care across diverse populations, including seniors, children, and individuals requiring specialized care such as dementia, hospice, or acute medical needs. The LPN will play a vital role in supporting patient health through skilled nursing interventions and collaborative healthcare practices.

Duties



Administer medications and treatments accurately following physician orders and established protocols Monitor patient vital signs and assess health status regularly Assist with patient admissions, discharges, and transfers Provide wound care, including dressing changes and infection control measures Support patients with activities of daily living (ADLs) and ensure comfort and safety Document patient information accurately using electronic health records such as Epic or Cerner systems Collaborate with physicians, nurses, and other healthcare professionals to develop and implement care plans Conduct phlebotomy procedures and assist with laboratory testing when necessary Educate patients and families about health conditions, medication management, and post-discharge care instructions Participate in case management activities to coordinate comprehensive patient services

Qualifications



Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) Prior experience in nursing home, hospital, or acute care settings preferred Experience with dementia care, senior care, hospice, pediatrics, or home health is highly desirable Knowledge of medical terminology, medication administration protocols, and pain management techniques Proficiency with electronic health records systems such as Epic or Cerner is a plus Strong patient care skills with a compassionate approach to diverse populations including children and seniors Ability to work effectively in fast-paced environments requiring attention to detail and adherence to safety standards Excellent communication skills for collaborating with multidisciplinary teams and educating patients and families

Pay: From $22.00 per hour
